My form is not working

My website URL is http://submit.epizy.com/try2/:

In this site (index.html), I have a form. The form will be process by contac.php.
But, when I click submit button (hantar in my website), the data is not sent to my mail.

I try with other hosting, and the data is send to my e-mail.
